莫德納(Spikevax)JN.1 COVID-19疫苗接種意願調查登記 (Spikevax)JN.1 COVID-19 Vaccination Willingness Survey Registration
為建立國人對COVID-19的群體免疫力，自110年3月22日起推動國人接種COVID-19疫苗，且因應COVID-19的病毒變異，持續提供新病毒株疫苗，以提升國人可對抗流行病毒株之免疫保護力，降低感染後發生之重症及死亡風險，積極維護國人健康，113年11月1日起開放6個月以上者為符合接種條件對象。由基隆市中正區衛生所安排三軍總醫院團隊入校進行疫苗接種作業。
To establish herd immunity against COVID-19 among the population, the government has promoted COVID-19 vaccination for the public since March 22, 2021. In response to the virus's mutations, updated vaccines targeting new variants continue to be provided to enhance the public’s immune protection against prevalent strains, reducing the risk of severe illness and death after infection, and actively safeguarding public health. Starting November 1, 2024, individuals aged six months and older are eligible for vaccination. The Keelung City Zhongzheng District Health Center has arranged for the Tri-Service General Hospital team to conduct vaccination operations at schools.

為準備充足疫苗數量，請有意願接種疫苗者進行下列資料登記，個資資料僅作為此次疫苗接種使用。In order to prepare a sufficient quantity of vaccines, those who are willing to be vaccinated are asked to register the following information. Personal information will only be used for this vaccination only.
如已完成「默德納JN.1新冠疫苗」接種者可不需再登記意願，敬謝！If you have completed the " Moderna JN.1 COVID-19 Vaccination " vaccination, you do not need to register your intention anymore, thank you!
